Job Summary:
The Care Coordinator will play a crucial role in ensuring a seamless patient experience by providing primarily clinical and administrative support. This position requires a professional with a patient-centered mindset, strong organizational skills, and the ability to communicate effectively. You will collaborate closely with our physician, Dr. Everett, and other team members to support treatment plans, coordinate care, and enhance patient outcomes.

Key Responsibilities:
  • Clinical Duties:
    • Conduct patient post-encounter process.
    • Resolve Inter-encounter clinical needs.
    • Assist in reviewing and preparing lab results for physician evaluation.
    • Support coordination and follow-up of treatment plans.
  • Administrative Tasks:
    • Primary contact for Concierge Patients, handling all their needs (20-30 patients).
    • Handle patient communications via email, phone, and online portals.
    • Maintain accurate and up-to-date medical records in EHR systems, including medications, supplements, lab records, and clinical documentation.
    • Supports the scheduling and managing patient appointments, including reminders and follow-ups.
  • Patient Education:
    • Provide information and resources about treatments, lab testing, and recommendations.
    • Address patient questions and concerns with empathy and professionalism.
  • Liaison Role:
    • Coordinate with laboratories, pharmacies, and external vendors, when needed.
    • Facilitate collaboration between the physician and other team members.
  • Additional Tasks:
    • Identify and implement improvements in workflows and patient care processes.
    • Assists with office maintenance as needed.
Qualifications:
  • Required Education and Experience:
    • Bachelor’s degree in health sciences, nursing, or a related field, or 1-3 years of clinical or administrative healthcare experience.
  • Preferred Qualifications:
    • Experience in integrative or functional medicine settings.
    • Familiarity with advanced testing (e.g., GI-MAP, DUTCH Test, mycotoxin analysis).
  • Certifications:
    • Certified Medical Assistant (CMA), Registered Medical Assistant (RMA), Registered Nurse (RN), Licensed Practical Nurse (LPN), or Community Health Worker (CHW) certification preferred.
  • Technical Skills:
    • Proficiency with EHR systems and online communication tools (Zoom, Slack, etc.).
    • Basic knowledge of medical terminology and patient care procedures.

Work Schedule:
  • Full-time position / Hourly-paid
  • Flexible working hours: Monday through Friday, 8 AM – 5 PM CST (1h lunch time included) (with some flexibility based on patient needs).
  • Occasional on-call or weekend requirements.
Compensation and Benefits:
  • Salary: Competitive and commensurate with experience. $25-30/h
