Your Role

  • Provide monthly calls and support to patients enrolled in the CCM program.

  • Utilize your nursing skills to educate patients and their families.

  • Monitor patients progress and work with the physicians to ensure quality patient care.

Qualifications

  • Active CMA Certification or Registration (nationally recognized)

  • Experience in geriatrics or primary care

  • Ability to sit and talk on the phone for long periods of time

  • System: We require you to have an i5 or newer Windows processor, and to have Windows Pro (not Home). This can be a laptop or desktop.

  • Must live/work full time in the US - you can work from anywhere within the US but not outside.

The Pay

  • The CMA pay ranges from $17.35 - $19.27 per hour. (40hrs/week)

  • The pay is a base pay of $15.00/hr plus a bonus of $1.00 per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
