Premier Home Dialysis Services provides in-home dialysis treatments to patients diagnosed with end-stage renal disease. Role Description

This is a full-time hybrid role for a Corporate Licensed Social Worker in Kalamazoo, MI. The Corporate Licensed Social Worker will work to lead LMSW teams across the United States. This will include but not be limited to policy and procedure creation, training, and on-boarding new LMSW in the company. In addition to the corporate role, you will maintain a caseload much lower than that of the large dialysis providers. 

It will include providing social work services to patients, including conducting assessments, developing care plans, coordinating resources, and providing ongoing support and education to patients and families. The Corporate Licensed Social Worker will also work collaboratively with the interdisciplinary team to ensure optimal patient outcomes.

Qualifications

• Clinical social work skills, including assessment, treatment planning, and crisis intervention
• Minimum Two Years in Dialysis 
• Driven to take the reigns and get things done
• Innovative ( doesn't believe the old way is the best way)
• Confident working remotely
• Able to Lead a team of LMSW to success
• Highly organized
• Knowledge of community resources and services
• Excellent communication and problem-solving skills
• Experience working with patients with chronic medical conditions
• Ability to work collaboratively with an interdisciplinary team
• Comfort using electronic health records and other technology platforms
• Master's degree in Social Work from an accredited institution and active Michigan licensure as a Licensed Master of Social Work (LMSW)
• Experience providing in-home social work services is a plus
