Director Case Management - Registered Nurse (RN)
Full-Time Position
$20,000 Sign-On Bonus and Generous Relocation Package Available!!!

Job Summary:

  • Oversee a team of case managers responsible for patient care coordination
  • Develop and implement case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiation
  • Evaluate patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standards
  • Seek treatment that balance clinical and financial concerns with the familial needs and patients' quality of life
  • Rely on extensive experience and judgment to plan and accomplish goals
  • Lead and direct the team
  • A wide degree of creativity and latitude is required
  • Typically reports to Senior Leadership

Qualifications:

  • Bachelor's degree in nursing or related field required, Master's degree in nursing or related field preferred

  • Current licensure as a Registered Nurse in the State of Oregon
  • Current BLS
  • Requires 3+ years of case management experience in an acute care setting
  • Requires 5+ years of clinical experience an acute care setting
  • Experience in process improvement and program development
  • Demonstrated competency in teaching
  • Supervisory experience preferred
  • Interprets standards of practice, benchmarks of care as related to care management in the acute care setting
  • Experience as a clinical preceptor
  • Manages specific populations as well as case management of individual patients
  • Effectively facilitates multidisciplinary teams and small groups
