HOURS: Full Time

Monday thru Friday 8:00am to 5:00 pm

JOB SUMMARY

Scope of Work:

Health Navigation is the process of advocating for and guiding a patient through the medical and behavioral health system. We provide one-on-one assistance with appointments, medications, and health literacy. The average case load is 1 navigator to 15-25 patients. Gellert Health provides our patients with holistic assistance from finding a healthcare provider, to understanding insurance coverage, and managing chronic conditions. By providing targeted support and guidance, we help our patients access timely and quality health care. The ultimate goal of our program is to ensure patients receive the best possible care.

Key Responsibilities

The Health Navigator will have a panel of vulnerable patients with an expectation of:

  • Ensure patients receive their prescriptions
  • Schedule all appointments with patient
  • Attend all appointments with patient
  • Keep track of patient’s medication refills
  • Document all appointment and patient information in an Electronic Medical Record
  • Relay information from one provider to the other
  • Help to prevent duplicative services and testing
  • Increase utilization of preventative services, decrease use of ED
  • Work day-to-day on patient goals
  • Connect patient to SDOH providers
  • Improve day-to-day safety of patients living environment
  • Transport patient to critical appointments in personal vehicle (generous auto-allowance for fuel and maintenance provided)
  • Fulfill additional responsibilities as assigned by Direct Supervisor
  • Provide Peer Support services

Assessment of Navigation skills and competency:

  • Weekly individual supervision
  • Ongoing monitoring of job performance
  • Quarterly Review & Annual Evaluation

Required prior to employment:

  • AZ Fingerprint Card
  • CPR/First Aid certification
  • TB Test/Flu Shot

PROBATIONARY PERIOD

The first 90 days of new employment or when accepting a new position shall be a probationary period and your employment may be terminated during this period at any time without notice during your first month, and with one week’s prior notice thereafter. A Gellert Health employee who accepts a new position in the company and fails at securing the new position may be offered a return to their previous position when available.

GH may, at our discretion, extend this period for a further 90 days. During this probationary period your performance and suitability for continued employment will be monitored.

New hires receive the following during the probationary period:

  • Extensive training and support from their supervisor
  • Weekly check-ins with the supervisor to receive feedback
  • Support for improving performance to meet our standards and expectations
  • A written performance evaluation at the end of the probationary period

Upon successful completion of the probationary period, the employee is granted permanent employee status. Completing this probationary period does not guarantee continued employment with Gellert Health. Employment is at-will and can be terminated at any time with or without cause. The employee is subject to all company policies and procedures.

REQUIREMENTS:

  • At least 21 years old
  • 39 months clear Motor Vehicle Record
  • Auto insurance
  • Ability to successfully pass a criminal background check. (Any convictions will be evaluated based upon the conviction in relation to the job requirements. A criminal conviction is not necessarily a deterrent to employment.)
  • Associate degree or bachelor’s degree, or work experience equivalent
  • Experience working with culturally diverse, chronically homeless, and vulnerable individuals, in medical and behavioral health services (preferred).
  • Clean and working four-door car
  • Current Arizona driver’s license
  • Bilingual – Spanish highly desirable not required.
