Job Title - Social Worker

Job Description

  • Attend annual review and department in services, as scheduled; staff meetings as required.
  • Support and maintain a culture of safety and quality.
  • Answer telephone calls professionally; refer patient problems to outpatient clinic physician; schedule appointments.
  • Assess the needs of individuals, families, and communities
  • Provide direct services such as counseling, support, and referrals to other resources
  • Develop and implement community based social work programs and initiatives
  • Advocate for policies and programs that support the well-being of communities
  • Build partnerships and collaborations with other organizations and community stakeholders
  • Conduct community outreach and education as a community outreach social worker
  • Provide crisis intervention services
  • Conducti research to inform and improve community-based practices
  • Collaborate with other professionals, such as healthcare providers, to ensure a holistic approach to addressing community needs.


Additional Skills & Qualifications

  • LCSW or LMFT - California License
  • Communicate clearly and concisely, both verbally and in writing.
  • Candidate will need to go through a 6-8 week credentialing program prior to starting
