HOME CARE NURSE

Home Care Registered Nurse will provide skilled professional service and case management oversight to a variety of home care patients. This is a great opportunity if you are looking for more autonomy, 1:1 patient care/case management within your community and enjoy having supportive management and work environment. We offer a 4-day work week option (8.5hr shifts) More quality time with patients 1:1 Responsibilities:

• Provide direct patient care in accordance with the individualized care plan
• Assist with activities of daily living, including bathing, dressing, and grooming
• Administer medications and perform treatments as directed by the healthcare team
• Monitor and document patient vital signs
• Assist with infection control measures to maintain a safe and clean environment
• Provide health coaching and education to patients and their families
• Ensure proper airway management for patients with respiratory conditions
• Collaborate with other healthcare professionals to coordinate patient care
• Maintain accurate and up-to-date documentation of patient care activities

Skills:

• Strong communication skills to effectively interact with patients and their families
• Ability to provide compassionate and empathetic care to individuals of all ages
• Knowledge of medical coding and documentation requirements
