Under the general direction of the Social Work Manager/LCSW, this role delivers age-appropriate social work care to patients and caregivers following hospital policies and regulations. The Social Worker II is an essential part of the healthcare team, providing assessments, coordination, treatment planning, and referrals to meet patients' and families' complex needs in hospital and clinic settings. The MSW will seek supervision from a Licensed Clinical Social Worker as needed and will use the Patient Medical Record and HPF systems.
