About Us: Meals on Wheels Orange County is the largest nonprofit provider of nutrition and supportive services for at-risk older adults in Central and North Orange County. Our mission is to combat hunger and loneliness among older adults through nutritious meals, friendly visits, and safety checks and to support families through day services. Annually, we deliver nearly 2 million meals to 20,000 at-risk older adults.

Organization: Meals on Wheels Orange County (Community SeniorServ, Inc.)

Location: Santa Ana, CA

Hours: Monday - Friday, 8:00 AM - 4:30 PM

Salary Range: $66,560 - $70,000 annually (Salary will be determined based on factors such as geographic location, skills, education, and experience)

Position Overview: We seek a dedicated and compassionate Program Supervisor to join our management team. Reporting to the Senior Manager, the Program Supervisor will manage program components established by the organization's strategic plan. This role involves program leadership and management, external relationship development, and knowledge management. The Program Supervisor will build relationships within the community to enhance resources and information available to eligible clients.

Key Responsibilities:

  • Monitor program performance, ensuring referral goals and targets are met.
  • Promote program benefits and identify potential funding sources.
  • Oversee case records, monthly statistics, and fiscal records.
  • Select, assign, direct, and evaluate department staff performance.
  • Coordinate with various entities to ensure seamless client experiences and prevent service duplication.
  • Manage relationships with tertiary care providers and referrals, enhancing continuity of care.
  • Screen clients for service eligibility and refer them to appropriate support services.
  • Conduct client-specific needs assessments, identify problems, and set immediate and long-term goals.
  • Arrange and coordinate a network of supportive services in line with client care plans.
  • Maintain records and reports in compliance with departmental, agency, local, state, and federal requirements.
  • Provide in-service education for staff and complete home visits.
  • Coordinate with hospital SNF staff on discharge plans.
  • Participate in meetings to share program information.
  • Perform other duties and special projects as assigned.

Qualifications:

  • Medical assistant certification preferred or related educational certification.
  • Minimum of two years of case management experience.
  • Experience working with older adults, elderly individuals, and people with disabilities.
  • Excellent communication, written, active listening, and interpersonal skills.
  • Thorough knowledge of case management principles and techniques.
  • Ability to maintain professional and confidential standards.
  • Strong problem-solving and critical thinking skills.
  • Working knowledge of programs and services available in Orange County for seniors.
  • Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
  • Strong understanding of cultural competency with the target population.
  • Must pass a background check.
  • Valid driver's license and insurance.

Physical Demands & Work Environment:

  • This position operates in a fast-paced, multi-tasking, professional environment, requiring ongoing learning and complex problem solving.
  • Ability to sit for extended periods and use both hands and fingers for typing.
  • Ability to operate office equipment such as copiers, printers, filing cabinets, and telephones.
  • Occasionally lift and move office equipment and materials up to 10 pounds.
  • Frequent computer use requires close vision.
  • The noise level in the work environment is usually moderate.
  • Occasional overtime required.
  • Local travel required up to 50% of the time.
