Work Setting: Field-based (home visits) with clinic collaboration

Schedule: Monday – Friday, 9:00 AM – 5:00 PM (1-hour lunch break)

Contract Type: Open-ended contract with the potential for direct hire

• This is a field-based role focused on in-home patient visits.
  
• The ideal candidate is a motivated self-starter who thrives in community-based healthcare and is committed to improving patient outcomes through tailored nutrition support.
  

Please note: Candidates must be willing to travel to the Bronx for initial orientation and occasional team meetings or training sessions.

Job Duties:

• 
  

  Conduct in-home nutrition assessments and develop personalized dietary plans.

• 
  

  Provide education and counseling to patients to support their health goals.

• 
  

  Track progress and adapt nutrition care plans as needed.

• 
  

  Collaborate with healthcare professionals to ensure comprehensive care delivery.

Qualifications:

• NYS Registered Dietitian
  

Experience:

• 
  

  Registered Dietitian Nutritionist (RDN), credentialed by the Commission on Dietetic Registration (CDR) .

• 
  

  Current Certified Dietitian-Nutritionist (CDN) license issued by New York State .

• 
  

  Excellent communication and organizational skills.

• 
  

  Ability to work independently and manage time effectively.

• 
  

  Experience working with diverse populations is a plus.
