Human Edge Agency – Job Opportunity Position Title: Registered Dietitian Nutritionist (RDN) Location: Flushing, Queens, NY Work Setting: Field-based (home visits) with clinic collaboration Schedule: Monday – Friday, 9:00 AM – 5:00 PM (1-hour lunch break) Contract Type: Open-ended contract with the potential for direct hire Number of Openings: 1 Company Information:Please note: Candidates must be willing to travel to the Bronx for initial orientation and occasional team meetings or training sessions.
Job Duties:Conduct in-home nutrition assessments and develop personalized dietary plans.
Provide education and counseling to patients to support their health goals.
Track progress and adapt nutrition care plans as needed.
Collaborate with healthcare professionals to ensure comprehensive care delivery.
Registered Dietitian Nutritionist (RDN), credentialed by the Commission on Dietetic Registration (CDR) .
Current Certified Dietitian-Nutritionist (CDN) license issued by New York State .
Excellent communication and organizational skills.
Ability to work independently and manage time effectively.
Experience working with diverse populations is a plus.
