*What You'll Do:*

Avanti Home Care RN’s work as a collaborative part of the health care team. Providing total client care to include; assessments, organizing caseload, coordination of client records, oversees field staff, medications and treatments administered as ordered by the physician, physical and psychological care, and client and family education. Assesses and interprets symptoms and reports significant changes in client's condition.

- Provide compassionate and personalized care to patients in the comfort of their homes.

- Collaborate with interdisciplinary teams to ensure holistic care plans.

- Educate patients and families on managing their health conditions.

*Requirements:*

- Current RN license in Wisconsin

Pay: From $32.00 per hour

Expected hours: 35 – 40 per week
